We implement administrative, physical, and technical safeguards compliant with HIPAA Security Rule (45 CFR Part 164, Subpart C) to protect your information from unauthorized access, use, or disclosure. Our staff receive regular privacy and security training, and we maintain policies to address breaches and unauthorized disclosures. - Data Breach Notifications
In the unlikely event of a breach that compromises your PHI, we will notify you within 45 days per Arizona data breach notification laws, as well as notify the U.S. Department of Health and Human Services when required under the HIPAA Breach Notification Rule. - Changes to This Privacy Policy
